Documents indicating that the appropriate insurance coverage, including indemnity for damage to third parties and for environmental damage or an adequate bank guarantee, trust fund, bond, line of credit, escrow account or such other form of security relating to damage to third parties and environmental damage, as the Authority considers appropriate, has been put in place |

| 3 | An emergency response plan which cover the movement of the waste from the point of generation/storage to the point of exit from Jamaica; and |

| 4 | Completed Licence Application Form (if there will be a discharge of trade or sewage and/ or poisonous or harmful substances into the environment | |
| 5 | A separate form must be completed for each intended export of hazardous waste except where such export is to the same foreign receiver subject to the form of operation for storage, recycling and disposal |

If company name is different from the name of the waste generator then : (i) Specify company name , registration number and country of registration
If company name is not different from the name of the waste generator then: (i) Applicant needs to determine whether or not generator of waste different from exporter

If generator of waste is different from exporter then: (i) Specify the name of generator of hazardous waste If generator of waste is not different from exporter then: (i) Specify the address and contact details for the generator of the hazardous waste (ii) Determine quantity of waste to be exported in kg/litres (iii) Determine and indicate the category to which your waste belongs according to First(1st) Schedule using the types stated there in (iv) Determine and indicate the port of exit from Jamaica
(v) Determine and indicate the port of entry in the importing State or transit State(s) if any
(vi) Complete application and affix signature(s)
(vii) Completed form must be signed by a Notary Public (viii) Complete from in triplicate (ix) Acquire and submit required accompanying documents to NEPA along with the application fee
