Step1 Indicate Permit /Certificate being applied for

Fill in personal and contact details 

Step 3

Determine and state the number of cases, degree processed and total weight in kg to be traded in in ach shipment 

Step 4

Determine and indicate the date of the proposed shipment 

Step 5

State the address of the premises/facilities where Queen Conch or opercula by product(s) is/ are currently stored

 

Step 6

Submit completed form with documents attached to NEPA
